Complete all sections. Print neatly or type. This information will be used by the faculty council to assist in the selection process. Completion of this information form does not guarantee selection. Selected students will have demonstrated outstanding performance in all four areas of scholarship, leadership, character, and service.

Upon completion, this information form is to be handed directly to Mrs. Young or Mrs. Rhynes. Completed forms will be accepted any time during regular school hours up to 2:30 pm on September 10th, 2008. Do not leave the form on the counter, do not give the form to any other staff members, do not give the form to a friend to turn in for you. Failure to follow directions and submit this form in the manner and time frame directed will cause the student to not be considered for selection.
