Reading records - TAB 1
Reading journal used to monitor daily reading during connections time - summaries written by students on a weekly basis, checked by coach with points awarded toward grade determination.
Reading log of books read over high school career - paperwork completed by students, checked by coach
Resume records - TAB 2
A record of accomplishments in preparation for resume and post high school applications (all maintained by student, checked by coach)
- Attendance records - provided annually at the beginning of the second term
- Awards/honors/certificates held
- Employment records
- Involvement in the school and community
- Transcripts - provided annually at the beginning of the second term reviewed by coach to guide the student during registration
- Volunteer work (Community service hours)
- WASL scores report - provided annually at the beginning of the year
Career exploration - TAB 3
- Career interest survey - counselors work with students to complete this survey. Students enter it in their portfolio and use it as a part of five year educational plan revisions and to determine possible post high school careers of interest. The survey should be reviewed by the coach to help guide the student. Parents and student will review the career interest inventory together during student lead conferences in the spring of sophomore year and declare top two areas of interest. One of the declared areas will serve as a career to be researched on line and presented to the connections group.
- Career research and exploration - a written summary which is shared orally with connections group as a graded activity. One per year, sophomore, junior, and senior years. The career of topic to be researched is guided by career interest inventory results.
- ASVAB results will also be included as a tool to guide the student into career areas of interest and ability.
- Written reflection of a job shadow, a minimum of one job shadow to be completed during the junior year. An oral presentation will be given to the connections group by each student, sharing job shadow results.
Educational plans - TAB 4
- Five year plan - completed by counselor/student/parent, reviewed by coach during registration.
- Student Improvement Plan - Created with state software in September once WASL results have been received. Improvement plans will be created for all students who have not passed any portion of the WASL. Plans will be placed in the portfolio and parents notified. Student improvement plans will be reviewed and signed by parent during student lead conferences in the spring. Those students who pass all portions of the WASL will not have an improvement plan.
Self-reflection and improvement goals - TAB 5
- Individual goals - students will write an academic goal and an involvement/social goal at the beginning of each school year. Reflective writing on goals will be completed at the end of the first, second, and third quarter.
- Self reflective essay - A summary paper written by students in May. The essay will be read by their parents with a section with written comments by parents. A self reflective essay will be written each year, with the senior year essay focusing upon plans for post high school.
Senior Culminating Project
Seniors should use their portfolio to keep copies of their culminating project paper work
