• Pfizer Vaccine (1st, 2nd or 3rd dose booster-must provide proof of eligibility for booster)

**If this is your 2nd or 3rd dose make sure to bring your vaccination card

 

COVID-19 Vaccine booster shots are available for the following Pfizer-BioNTech vaccine recipients who completed their initial series at least 6 months ago:

• 65 years and older

• 18+ who have underlying medical conditions

• 18+ who work in high-risk settings

• 18+ who live in high-risk settings

Parents/guardians must accompany children ages 12-15. Parent/guardian permission is required for 16 and 17 year olds to receive the vaccine. *If you are unable to attend with your 16 or 17 year old child, please pre-register them online (link below), checking the permission box to allow the child to receive the shot unattended. You will be contacted that day for verbal verification.

 

 

 

Note: If an East Valley staff member or student has been fully vaccinated for COVID-19 and is considered a “close contact” at school, and they are not exhibiting any symptoms, they will not be required to quarantine and miss school. It is recommended they test within 3-5 days of suspected exposure.
